RISK ACKNOWLEDGEMENT WAIVER

THIS IS AN IMPORTANT DOCUMENT. PLEASE READ CAREFULLY BEFORE SIGNING

Minimum Height 1 metre. Maximum participating Weight 120 kg/ 19 stone. Minimum Age 7.

In consideration of my participation in the course, I acknowledge and agree to the following:

I am aware of the age, weight, height, and medical restrictions and confirm that I am in reasonably good medical condition. I understand that failure to abide by these restrictions can create a significant increase of risk of harm to myself, other participants, and employees of Zipit.
I understand that magnets used in the safety mechanisms may interfere with the operation of pacemakers and ICD’s.
I understand that medical conditions (including pregnancy) may be worsened, or increase the chance of injury, by participating in the course. I certify that to the best of my knowledge neither I nor any under 18 year olds in my care have a medical condition which might make me or them more likely to sustain an injury. To the best of my knowledge I and any children in my care are not pregnant.
I agree that contact with instructors is necessary whilst being fitted with a harness or during assistance.
I agree that the harness and gloves must be worn at all times and must not be removed or shared with any other person. Any costs incurred due to intentional misuse or damage of equipment, will be borne by myself.
I understand that Zipit is a physically demanding activity and I am responsible for my own safety and that of any children under my supervision. I understand that participation in the activity exposes me to certain risks and that there will be a risk of injury when undertaking it. For example, legs can be susceptible to injury on zip wire landings, fingers are vulnerable to equipment if not used correctly, teeth are exposed to metal clips if not used correctly.
To reduce the risks, I agree to follow all the instructions set out in the Safety Rules during booking and at the cabin, along with all instructions to be given to me by Zipit staff during the Safety Brief and during the activity. Anyone in breach of our rules and regulations will be asked to leave the course immediately (without refund). Anyone deemed to be under the influence of alcohol or drugs will be refused entry.
If assuming responsibility for the supervision of any under 18 year olds in this agreement I declare that I am aged 18 years or older. In order to control behaviour I will remain in visual contact of the children in my care for the duration of the activity. I agree that I am responsible for any under 18 year olds in my care and undertake to ensure that they read, understand and will follow the instructions set out in the Safety Rules, the Safety Brief and from staff during the activity. I declare that if I am not their parent or guardian, I have the authority of the parent or guardian to sign this risk acknowledgement and disclaimer.
In the event of an accident, or loss or damage to personal effects to myself or the children in my care, I acknowledge that Zipit will not be liable for any direct or indirect loss, damage or injury arising from or in connection with the activity (except death or personal injury caused by Zipit’s negligence) and I waive all and any claims against Zipit in this respect both for myself and/or for the under 18 year olds in my care.
I confirm that I am not under the adverse influence of drink or drugs.
If I am 18 years of age or older, I grant to Zipit the right to take photographs/videos of me in connection with my participation in the Activity, and convey all right, title and interest in and to the same to Zipit. I authorize Zipit to copyright, use, and publish the same in print and/or electronically, and agree to its use for any lawful purpose, including but not limited to publicity, illustration, advertising, and Web content.

SUPERVISORS OF UNDER 18'S - ADDITIONAL INFORMATION

A Parent or guardian must sign on behalf of the child attending and by signing they agree that their child will be supervised at all times by a responsible person over the age of 18.

Active Supervision must be provided at all times. Adults must remain on same circuit as children, if participating, and must closely monitor their activity throughout the duration of the visit. Non-participating supervisors must be able to see all of the children under their supervision, all of the time, from the ground.
All supervisors must listen to the safety briefing along with the children.
A copy of the company’s Child Protection Policy is available on request.
